Coordinated Entry (CE) is a system designed to increase the efficiency of a homeless assistance system by enabling access to homeless services while coordinating program referrals. Ravenwood Health acts as the point of entry for those experiencing homelessness into the Homeless Management Information System (HMIS). These programs include, but are not limited to, Permanent Supportive Housing (PSH) and Shelter Plus Care (SPC). https://cohhio.org/
Community Residence (CR) is a Ravenwood Health program that provides a housing opportunity to those in need. These are shared-living apartments which we are available and managed through Ravenwood for adults and families of Geauga County. You are not required to be a Ravenwood Health client to participate in this program. We are readily available for those in need of housing supports.
